Medication Permission Slip

To the Parents:

Welcome parents to a new school year. Hope everyone had a fun and relaxing summer. It’s school time again and I would like to take a moment to remind parents of a couple of things.

First, please remember when sending ANY type of medications to the school for your child to send it in its original container with the students name on the front along with a note giving permission and the time and dose that is to be giving. If possible, please fill out one of our permission forms. If not available a hand written letter will also work. This includes prescription medicines as well as over the counter medications. While most of us don’t consider cough drops as being medication they are and we need them placed in a bag with the students name on it along with a note from you to give us permission to give your child this medicine.

Second, flu season will be knocking at our door before we know it. Think about having your child vaccinated for the flu now to avoid sickness later. Remember the district rules regarding illness. Don’t send your child to school with a temperature. They have to be fever free for 24 hours without taking any medication before returning to school.

The attached medication permission slip may be kept for future use and duplicated as needed. Wishing everyone a good illness free school year. If you have any questions don’t hesitate to call.
